I need to approve the user, after their registration in site.
I can see the option for that in General Setting, User Verification.
And the options are 1) Email Verification 2) Admin Verification 3) Off
Which requires new registrations to click a link in the notification email to enable their account.
The option is showing always in off, even after i change to option 1(or)2 and update.
Some times its showing blank page, when I refresh again, its back to normal with the same option (3).

Its looks weird, what version of PR you are using?
So, except for this option, all other changes taking placed or everything on the General Settings setting back to its default options?
Also please make sure you have “Delete all settings on Deactivation” option unchecked or off (it still not have any relation with the option saving – except when u deactivate and reactivate the plugin).
Also please let us know what version of PHP and wordpress you have installed.

Thanks for the updated info. Well, the current version is 2.0.21, many things have been changed since then, so we can not give you any sort of guarantee for the older version still work.
I would recommend you to make a clone of your site and update the plugin there, if everything work fine then move to your live site.

I have a close to similar issue, am using WordPress 4.4 and pie-register version 2.0.21. The settings are ok but no verification emails are being sent. Am using multsite wordpress and initially thought it was the problem but I tried out with another site that’s not a multsite but it didn’t work. Emails with other plugins are working properly. Am using mysql version 5.5.45. Any assistance will be highly appreciated.

PR replies on WP_MAIL function, so if WP_MAIL working on your site, it should send you emails. In most of the cases users get fixed it by installing an SMTP plugin.
