forgot password page doesn't work

After receiving a password reset email (with the link http://cre8tives.org/login/?action=rp&key=U9JGUS4aG7xRphpnPSl4&login=future)
It directs to the correct login page, but then the shortcode doesn’t function anymore… it appears in the page as if it was broken.

Please make sure you put the shortcode in HTML mode of your editor also
please make sure PR is activated on the site.
